THE RICH AND THE REST OF US: A POVERTY MANIFESTO By Cornel West and Tavis Smiley â€Å"Our intent with The rich and the rest of us is to make [people] think about the pervasiveness of poverty, its real causation, and the threat it poses to our democracy. We want to raise awareness about poverty and discuss how best to end it- in out lifetime. â€Å"(Excerpted from the introduction, page 10-11) When I started with this book, I had expected it to be a book on how depressing poverty is and maybe how to become rich. What I came across was none of that but a realistic approach with some new ideas of how to deal with poverty.The authors have explained everything in simple language that is discernable by everyone and no necessarily economists. It is based on the US economy and not poverty all over the world. It deals with the situation of poverty in the US. Recently with the Presidential election campaign, a lot was said about the difference between the middle class and the rich. Especial ly with the tax cuts issue where tax cuts are being wanted for the middle class and the wealthy shall not be given any cuts. Any household that is earning anything more than $1 million per year should not be paying lesser income tax than what a middle class family does.This book has historical backgrounds, personal stories and statistics to say to us that poverty is not just a big issue but also a national threat along with bringing shame upon USA. It seems that the citizens of America as a whole does not choose to solve this problem. Cornel West and Tavis Smiley are crying out for help to speedily address the growing poverty before it forever distorts the US democracy, values and economic future. The great recession has affected a large number of people who have been forced into poverty and also deepening the poverty that already exists.United States census declares that 50% of all Americans are now in the low income or close to low-income bracket. When the biggest economic institu tions fell due to the recession, they went to the Government for help, which was given to them. But, in turn, the majority of Americans suffered. There were no more jobs and place to live. One-third of the American middle class has fallen into poverty. This book is a fast read that encapsulates current issues with the historic past and obvious advocacy. This book shows how every president has handled the problem of poverty and the change of political and popular attitude towards the poor.They also talk about a â€Å"new poor† that has been added to the homeless, food insecurity and unemployment. They use Martin Luther King as a major example who immersed himself in the subject of poverty before his demise. Though, he saw it as an issue that only affected the African-American population. The new poverty is not like the poverty we associate with wrongdoing. This new poverty is the â€Å"working poor† who have jobs and sometimes more than one person in d family is earning . Just they are not being paid enough to sustain themselves and their families.They have no unions to protect their interests, no benefits like medical or retirement and long working hours. There is no surety of jobs and no guarantee so every worker is one salary away from poverty and also one lay off away. Basically Smiley and West has started a War for the poor. They are trying to give a voice to those less fortunate. For them, their ultimate goal is to try and give back the American dream, which has now been down sized and outsourced. They say that during the presidential elections on 2012 both the parties never said the word poor or poverty.Another important aspect of the book is the Occupy movement. It was what marked the state of their inequality in economics and an ongoing change in America’s policies and priorities. Tavis Smiley says in the book â€Å"We wanted people who are struggling in this current economy to know that they are not alone and not forgotten. † It tells us about a imperfect system where there is no economic equality. Even though America is changing into a nation of major minority, it does not have the market unity to ensure success. The poverty manifesto talks about twelve points about what steps to be taken to help the situation.First and foremost fair wages should be given to the people so that they are enough to live on. Then the Government needs to introduce more jobs; the prison and mass incarceration rates need to be looked at, tax codes, fair inflation rates. The most imperative of all is to release a call for a conference at the White House on the abolition of poverty and for that a sample letter must be sent to the President. This book says that USA is in a state of emergency, and that they should stand up and take some action. They need to work towards sustaining solutions and jobs with living wages.Unless they do that, they will not get the status of a great nation back. The efforts have to come from bottom up and will need brave leadership at the top most level to get back America in old form. Politics is run by money, and people with substantial amounts can actually affect the public policy. It is how the democracy of USA was designed and not much has changed since it was founded. The only major difference is that now corporations influence more than individuals now. The power of money is the same no matter whether you are a republican or a democrat.I find that the first part of the book is very good. It gives us a good look into poverty and backing it up by statistics and graphs. West and Smiley delve in deeper and examine the emotions comprised in poverty. I enjoyed the way it is written and how the authors provide us with two different voices. Personally I think that the book lacks where the solutions are concerned. No doubt hay have given some ideas on how to take on the problem but most seems repetitive and doesn’t seem like they are thorough. After reading the book I have a much better understanding of the poverty situation in America.I had heard the presidential debates and the speeches but never really understood the seriousness till I read this book. This book is highly recommended for all those to want to face problems by smart public policy, which would end up getting work for people and also providing the structure and services needed. A new economy can be created that will be supportable and progressive. One that will put the citizens on America back to work, just, the right leadership is needed. Every day people get ill from the food they eat. Micro-organisms including bacteria, viruses and moulds found in food can cause food poisoning, leading to a whole host of unpleasant symptoms, such as stomach pains, diarrhoea and vomiting. Food poisoning can sometimes lead to gastroenteritis (inflammation of the stomach and bowel), or more serious health problems such as blood poisoning (septicaemia) and kidney failure. Anyone can get food poisoning but some people, including babies, children and older people, are more likely to have serious symptoms. This is why this is vital as in the Seaview nursing home the residents are frail older people and several also are in poor general health. They are many basic hygiene practices for food preparation and cooking. This includes: Surfaces Equipment Sell by dates Food Cooking storing Surfaces Food poisoning affects thousands of people every year and many of these cases go unreported. The exact numbers of cases of food related illness that occur each year are unknown. This is because many people do not seek the help of their doctor when their doctor when they develop symptoms. Food poisoning can be extremely dangerously especially for the very young, older service users and those who are ill or infirm. It is therefore important that care workers actions do not increase this problem. To prevent contamination of food it is necessary to follow recognised hygiene procedures to make sure that risks are kept to a minimum. Cleaning and disinfection chemicals are available to maintain the correct standards of cleanliness. Usually these chemicals are added to water to make a cleaning solution. Some come ready to use in spray or aerosol bottles for convenience. Substances used for cleaning food preparation areas fall into three categories: Detergents – are chemicals that will dissolve grease and assist the removal of food debris and dirt. Detergents do not kill bacteria. Disinfectants – are chemicals designed to destroy bacteria and reduce them to a safe level. However disinfectants are not effective at removing grease and dirt’s. Disinfectants have a very strong smell and have the potential to taints foods and if in contact with surfaces that food is prepared on extra care should be taken in rinsing with hot (above 82C) water. Sanitizers– are a combination of both detergents and disinfectants and are designed to kill bacteria and remove dirt and grease at the same time. image00.pngimage00.png Disinfection can also be carried effectively using heat i.e. using hot water (above 82C). Chemicals can be used in addition to this process however are less effective where hot water is not available or a preliminary clean had not been carried out. The combination of the above chemicals and very hot water provide an effective way of keeping surfaces clean. Any work surfaces that is used for food preparation must be strong, durable, easily cleaned, resistance to stain or to absorbing liquids and not easily damaged. In professional food areas stainless steel tables are used for food preparation activities. They are usually on wheels which have brakes so that they can be moved easily which allows for ease of cleaning. It is vital that work surfaces are kept clean and bacteria free. The clean as you go method should be used. This means that all equipment and surfaces should be cleaned as soon as they have been used. This describes cleaning that is carried out as soon as the mess is created, the aim of this is to prevent cross contamination and to keep work areas clean and tidy. Examples of this might be Washing and sanitising of a chopping board straight after use and Cleaning up a floor spillage immediately after it has happened. Any spills should also be mopped up immediately they occur. Work surfaces should be left clean and clear when work has finished. The 5 stages of cleaning work are: Remove food particles and spillages using a suitable damp cloth Use a hot water and detergent solution to remove any grease and debris Rinse thoroughly with very hot water (82C) Use a suitable disinfectant to reduce bacteria to a safe level Final rinse with hot water and dry using paper towels (or allow to air dry) Equipment Only clean equipment if you have been trained to do so. This is especially important where the equipment has sharp cutting surfaces and or moving parts that are run by electricity. Care should be also taken with equipment that is hot or generates a lot of heat, for example cookers and deep fat fryers When training has been done some basic steps should be followed when cleaning equipment. The basic steps are: Disconnect the equipment from the power source Take extra care when removing any blades Remove all waste food Thoroughly wash and sanitize all parts Reassemble the equipment taking care to fit all components correctly in case they fly off during use Sanitize again those parts that will come into contact with food Ensure that all safety guards are refitted correctly. All other small items of equipment like pots, pans, cutlery, plates and glassware can be cleaned in the usual way. This may either be in a dishwasher or by hand using detergent and hot water. Sell by dates Perishable is foods that are likely to go off quickly. They normally have a sell by and use by date on them. Selling food after its sell by date is an offence. The food can be used up to and including the use by date. For an example a carton of milk may have a sell by date till the 14th march and the use by date may be the 16th march. The milk can be used on the 16th and before but not after. Most other foods have a best before day; this indicates the month up to which it will be in its best condition for eating. Some foods don’t have to require a date for example fresh fruits & vegetable’s and meat from the butcher. Food It is vital that certain precautions are taken when providing food for service users. Washing of the hands is vital so hands must be washed: Before entering a food area After using the lavatory Between handling different types of food such as raw meat an cooked Before and after touching foods After coughing into the hands or using a handkerchief After touching face and hair After carrying out cleaning or handling rubbish It is essential that this is done because many bacteria live on the surface of the skin. Many of these are harmless however some can cause illness. Bacteria can be acquired from other sources and can contaminate food. Handling raw mean and poultry and then handling cooked meat is very dangerous unless hands are thoroughly washed in between. Touching the nose or coughing and sneezing over food or preparation areas should be avoided. This is because personal cleanliness is essential or bacteria will be transferred to the food. Also avoid touching the food with hands and glove wearing or using of the tong when possible is a good thing to do. This is because the less hands are in direct contact with food, the less chance there is of contamination. Touching dishes or cutlery that is to come into contact with food should also be avoided. This is because this cuts down the transfer of bacteria. Keeping the hair covered with either a hat or net and not combing hair in the food area should also be done as hair and scalp can carry bacteria that can fall into the food. Also keeping cuts and grazes covered with brightly coloured dressing as wounds are infected with bacteria and if the dressing comes off it can be easily found. Not smoking in the food areas is vital as it is against the law and can contaminate food. Also when ill food handling should not be done. This is vital because a person who is ill can infect food. Another thing that should be done is wearing clean protective clothing as there are fewer bacteria on clean clothes. Cooking Heat kills bacteria and this is why food must be cooked thoroughly. Cooking food at temperatures over 70Â °C will kill off any bacteria. If food isn’t cooked at a high enough temperature, bacteria can still survive. Take special care that you cook meat all the way through. Unless you’re cooking steak or lamb and beef joints rare, it shouldn’t be pink in the middle. Use a clean skewer to pierce the meat. If it’s cooked properly, the juices will run clear. If you’re cooking meat so it’s rare, make sure that it’s properly sealed (browned) on the outside. Large pieces of meat take longer to heat up to the centre. The need for sufficiently high temperatures reaching the centre is very important. Always re-heat pre-cooked food thoroughly and only do so once. When cooking food in the microwave, stir it well from time to time to ensure that it’s evenly cooked all the way through. It is bad practice to mix previously cooked food with newly cooked food. Topping up soup lowers the temperature and increases the risk of bacteria growing. It is much safer to make up food in smaller quantises as and when it is needed. Eggs can carry salmonella, so to safeguard service users, eggs should be cooked for around seven minutes at 70ËšC. High risks foods such as eggs and chicken that are eaten immediately after cooking are safe providing the temperature is high enough. If there is going to be a gap in time between the foods being cooked and being eaten, it must be kept hot. Equipment should be used to hold the food at a temperature of 63ËšC or above. This is could be a heated tray, trolley or service counter. When using this type of equipment; heat the equipment to at least 63ËšC before loading the food, ensure the food is already fully cooked and at a minimum temperature of 63ËšC and never use the equipment to heat up cold or partially heated food. Reheating food can increase the risk of food poisoning. Some food handlers often make the mistake of thinking that because food has already been cooked it is free of bacteria. They believe that is only needs warming up. If it happens, bacteria had ideal conditions for growth for example food, warmth and liquid. Guidelines for food that needs reheating is as follows: Don’t get the food out of the refrigerator too soon and leave it lying it around Handle as little as possible and keep it covered Divide larger items into smaller portions where possible Heat the food to at least 70ËšC as its core Serve quickly following reheating Never reheat cooked food more than once If reheating ready-made meals from a shop, follow the manufactures instructions in addition to the above advice. Storing If you don’t follow the storage guidelines that come with your food, you could be letting yourself in for real problems. Storing food in the wrong place or at the wrong temperature can lead to the growth of bacteria. This means that even one piece of food left out for a couple of hours can contain millions of bacteria as bacteria flourish at body temperature which is 37ËšC. The reason foods are stored in a refrigerator is because bacteria and viruses can’t multiply below 6ËšC. The correct temperature for a refrigerator is between zero and 5ËšC. Chilled foods should be put away quickly but don’t overcrowd the refrigerator as it raises the temperature. With cooked food, let steam evaporate first before covering and placing in the refrigerator. Always keep the refrigerator clean by washing the inside surfaces with warm soapy water. Never thaw then refreeze food. To slowly defrost, take the dish out of the freezer and leave it in the refrigerator overnight rather than on the counter top. Place meats on a plate on the bottom shelf. When defrosting with the microwave, cook the dish immediately. Never freeze food that has gone beyond its use by date and remove old food regularly. Keeping the refrigerator at the right temperature helps prevent bacteria from multiplying. The correct temperature for a refrigerator is below 5ËšC and a freezer and should kept at minus 18ËšC. Check both the refrigerator and freezer regularly with a reliable thermometer. To maintain a constant temperature, keep the door shut whenever possible. When storing food in a fridge: Don’t allow the juice from the raw meat, fish and poultry to spill or drip onto any other foods. Put fruit and vegetable’s items into the salad drawer Keep milk and fruit juices on the bottom rack in the door Place dairy products , dressings, spreads, sauces, cream and convince foods on the centre and top shelves Store raw meat, fish and poultry on the bottom shelf of the refrigerator. this prevents cross contamination as the blood as the other liquids from the meat and fish cannot drip down onto other foods Cooked foods should be kept on the top shelves of the refrigerator Mayonnaise and ketchup should be kept in the refrigerator after opening Never place food in an open tin in the refrigerator. Use secure containers instead and throw the food out after two or three days Keep seafood either in the refrigerator or in the freezer until you are ready to prepare it Cross contamination occurs when food is put in contact with other contaminated foods. An example of this could be the blood from a piece of chicken dripping onto a plate of cooked food. Cross contamination means that the bacteria or other pathogens have been transferred from an infected food item. They either become infected or contaminated. If food looks or smells in any way, throw it out. A sure sign of spoilage is mould. Most mouldy foods should be binned along with leaking cartons and food which has gone past its use by date. Most frozen foods should be thrown out after three to six month’s. When storing food in the freezer, remove it from the wrapping and place it in a labelled and dated freezer bag. Remember to expel all the air from the bag before sealing it. If a power cut has occurred then foods that have started to defrost should be thrown out. If the power comes back on and if there is any doubt about which foods have defrosted and refrozen, the food should be thrown away. Food that does not need keeping cold should be kept in a sealed container in a cool dry cupboard or larder. It should never be stored on the floor. Vegetables can normally be kept in a cool, dry place in loose bags or racks but not in plastic bags. This makes them sweat and rot quicker.

Jack London and His Call of the Wild Essay

â€Å"He was sounding the deeps of his nature, and of the parts of his nature that were deeper than he, going back into the womb of Time. † – Jack London, The Call of the Wild, Ch. 3 (Jack London Quotes). This quote summarizes the success of Jack London’s writing career in one simple sentence. London’s success and inspiration for his naturalist style can be accredited to the way in which he was raised, and his experiences during his lifetime. Jack London, was born John Griffith Chaney on January 12, 1876 near San Francisco, California. His mother was abandoned by London’s real father, William Chaney a traveling astrologer, shortly after it was discovered she was expecting Jack. This later influenced London’s decision to leave his family at a young age. His mother remarried quickly, and Jack took on his stepfather’s name, London. Because of complications, London was primarily raised by Virginia Prentiss, a former slave, until he was around five years of age (Stern 700). The family lived in poverty, and he had many siblings, but was not particularly close with any of them (Jack London Biography). At age 13, he quit school, borrowed money for a boat, and began harvesting oysters in the Pacific Ocean. By the age of 15, Jack was a successful business man, and known around the docks as â€Å"The King of the Oyster Pirates† (Stern 700). He later reflected on this difficult time by stating, â€Å"Life is not a matter of holding good cards, but sometimes, playing a poor hand well† (Jack London Quotes). He was later caught in this illegal act and then hired as part of the coast patrol because of his extensive knowledge of the sea. After a voyage to Japan with the patrol, he returned to California in the middle of a recession (London, Jack). After struggling to find a steady job, London joined Coxey’s army, a hobo organization (Jack London Biography). After becoming bored with that, he enrolled in high school and completed a four year degree in just one year. Shortly afterward, London enrolled at the University of California on borrowed money (Jack London). While attending the University he spent large amounts of time in the school’s library reading the vast collection of books. Among these were works from Friedrich Nietzsche, Karl Marx, and most importantly, Charles Darwin. Influenced by his readings, London created his own fusion of socialism, male dominance and white superiority beliefs (London, Jack). In fact, some experts even refer to his books, White Fang, and The Call of the Wild, as fiction versions of Darwin’s Evolution (Stern 700). In the middle of London’s college career, the first Klondike gold rush began. In 1897 alone, over 30,000 men, London among them, rushed to the Yukon territory in search of the precious metal. The majority, like London, returned unsuccessfully. Although he didn’t get rich on gold in the Yukon, he would later be rich on something else from the cold, harsh place; his memories, inspiration for two of his most successful novels, White Fang and Call of the Wild (Yukon). Around the turn of the century, Robber-barons as they were called, monopolized businesses, companies, then entire industries. Jack knew this fact, and believed that writing was his only way out (Jack London). London’s very first writing success Typhoon off the Coast of Japan, inspired by his trip to Japan, came in 1900, the same year he married his first wife, Bess Maddern. Together they had two daughters, Joan and Bess (Jack London Biography). During his first marriage, London published some of his most successful stories, including Call of the Wild (1903) and Sea Wolf (1904) both in which he dramatized â€Å"atavism, adaptability, and the appeal to the wilderness† (Jack London). In 1905, following an affair, London divorced Bess Maddern and married Charmair Kittredge. He later used his second spouse’s character as the protagonist in many of his works. London encouraged Charmair to pursue writing, and with his help, published three books, including a biography over Jack because of his eventful past (Stasz 1). Later that same year, London found his true love, Beauty Ranch. Later in his life he stated, â€Å"I write for no other purpose to add the beauty that now belongs to me. I write a book for no other reason than to add three or four hundred acres to my magnificent estate† (Jack London Quotes). London’s only true love perhaps, was the wilderness, traveling, and the great outdoors. In his later years he was well traveled, visiting Japan, Canada, Cape Horn, Australia, and even Mexico (David 1). London had nearly completed his dream house on Beauty Ranch when it mysteriously burned down; arson was suspected. From then on London’s health slowly but steadily declined. Jack London died on November 22, 1916 due to Kidney failure (Stern 700). Jack London is often considered the first American author to gain international fame through his fiction works alone. Although a very successful author, even in other countries, London’s books also received harsh criticism. His ideas and concept were said to, lack consistency and precision. He also struggled with other thoughts. He wrote and supported women’s suffrage, yet believed in male dominance. London supported white superiority, yet thought it was shameful that, the reckless white men destroyed the cultures of the natives he had seen while searching for gold (Stasz 1). Despite being known for his novels, London also wrote a few non-fiction books, and many short stories, his most famous being To Build a Fire, inspired by his own adventures in the Yukon (London, Jack). Although London went through many adventures, he did not credit these to his inspiration. When asked where he received his inspiration he answered, â€Å"You can’t wait for inspiration. You have to go after it with a club† (Jack London Quotes). Jack Londons writing’s are some of the most famous works of naturalism of all time. They have been described as, â€Å"works that deal romantically with elemental struggles for survival. He is one of the most extensively translated of American authors† (David 1). Jack London was a go-getter. Although he died at a young age (40), he accomplished much during his lifetime. The proper function of man is to live, not to exist. I shall not waste my days in trying to prolong them. I shall use my time† (Jack London Quotes). London’s success and inspiration for his naturalist style can be accredited to the way in which he was raised, and his experiences during his lifetime. For without these, London would’ve had nothing to write about. Because of the role fate played in his life, London grew to become one of the most successful authors of all time, and his books continue to sell in our modern day and age.

It is critical that people and the organization are in complete harmony to fulfill this purpose. Â  The most valuable assets of any business are its people. This is one fact that is singularly recognized at Southwest. Therefore they have a created a People Department instead of the HR department to give it a more humane face. Whereas in the aviation industry and indeed in other industries too, Human Resources represent a professional, structured and therefore very stiff-collared concept, at Southwest it means entirely the opposite.

They have a dark setting in some parts of the film and involve the destruction of a specific group. In the film the golem, there is the use of the creature golem, created to protect the Jewish people against persecution. A golem is a creature made from inanimate matter, an anthropomorphic being that is animated. The film is about a rabbi who creates the golem from clay and brings it to life using magic. In the end, the creature turns against them and sets fire to the ghetto. A little girl stops the creature by taking away an amulet from its chest in the shape of a pentagram. Men in black, involves two men wearing black suits in the pursuit of any alien related activities around. They are in pursuit of a bug leader in search of a device called the galaxy that will make tides in favor of their existence. Their job is to eliminate any alien forms and to ensure that no one remembers what they see if they come into contact with these creatures. They use a nueralyzer to wipe out people’s memories. In the end of the film shows a big fight between the alien form and the humans. The golem uses more sound effect and expressions while the men in black uses a lot of prop and camera angles. The use of micro elements is visible in each of these films. The use of mise-en-scene through costume and make-up, staging, setting, figure expressions and props is available in both films. Cinematography shows us how lighting has been implemented to produce a certain effect, the use of photographic elements such as the position of the lens and the camera angle. There is also the framing in a scene, special effects and composition. We have performance, which is the use of vocal delivery, physical expression, and an interaction between the performers. For a film to have proper organization of time there needs to be a little editing. Organization of time is both in a sequence and across segme nts of the story and the space organization. It helps in creating consistency for the viewer. The use of sound includes diegetic and non-diegetic sound and the use of different aural elements to create visuals (Appleman 12). All these elements display in the films the golem and men in black. â€Å"Men in black† uses different props and costumes to represent the alien forms. The golem on the other hand shows us a giant creature made out of clay that comes to life through sorcery. In the golem, we see the creation of the creature by a rabbi to protect the Jewish from persecution. He creates it from clay and uses sorcery to bring the creature to life. With time however, the creature turns against his creator and his people and sets fire to the village and kidnapping the rabbi’s daughter. The people scream for help and get the rabbi to cast a spell and stop the fire. The script reads, â€Å"Save us rabbi, recite the fire spell or we shall all perish†. They watch as the rabbi gets inside the ghetto and stand on top of a hill to reciting the spell. The people bow down in prayer with the rabbi. We see the golem dragging the rabbi’s daughter towards the staircase and set her below them. He then leaves her and heads towards the door. Dealing with angry and/or agitated customers is probably the hardest part of the job. Not only is it awkward, but it is also very agitating for you. I normally want to just scream in their face that it’s not my fault but I always keep my cool. Working at a fast pace without stressing is also a very good skill to have. You have to be able to work very fast at In N Out. We are constantly busy and having to please hundreds of customers a day. B. What are the possibilities for advancement at this current job, and what are the skills you could still learn at this job? * In N Out runs off of a level system. A level 1 cleans the dining room. A level 2 takes orders and can work the back pay window. A level 3 can take orders outside with something we call a hand held, and can also hand out food for the drive through. A level 4 works fries. A level 5 dresses, puts the condiments, on the burger. A level 6 actually cooks the burgers. Lastly, a level 7 is a manager’s assistant. They help run the shift when the manager isn’t around. Then after all those are the managers. Each separate store has 4 different mangers. They move up as well. They all start out as 4th managers and then move up to a 1st manager, which is a â€Å"store manager†. After that they can move up to district managers and then a regional manager. I am currently a level 3 but I am hopefully getting my level 4 soon. I’ve been working and learning fries. In N Out is definitely my fall back if the ultrasound technician fails. Step 3: Interests A. What do you like to do? * I love to hang out with friends, my boyfriend, go out to eat, and sleep. I can hang out with my friend and Jason for days. Probably like every other teenager but that’s when I have the most fun. Going out to eat is my favorite thing to do. I don’t really care if it’s bad for you. I love food. Sleeping is what I do in my free time. I am constantly tired so I love sleep when I actually have the time to do so. B. What is the best experience you have ever had? The best experience I’ve ever had was probably my senior prom. Basically it was the best night of my life. It was amazing. From getting ready, to pictures, to dinner, to the bus, to the dance, to the bus again, and then Kyle Wades spa. I got to get ready with my best friends and my boyfriend’s mom paid for me to get my makeup done. I love that woman. The pictures were great with m y boyfriend and best friends. We got some really cute ones. Dinner was delicious at chilis. The bus was the best part in addition to the actual dance. We had a stripper pole and all the guys got on it. This is the irst dance I’ve been to that the teachers didn’t care how we danced. By the time we got off the dance floor everyone was dripping sweat. My legs were sore for 3 days after. After that we hopped back on the bus, made a quick stop to In N Out and then chilled in Kyle’s spa. Jason and I had to sleep in my truck that night but we woke up early and went to Denny’s for breakfast. C. Quality and Risk Scenario Essay The HIM professional can have a direct impact on the quality or compliance of specific operations or employees within a healthcare institution. Healthcare institutions, such as hospitals, can be huge institutions made up of hundreds or even thousands of treatment or operation specific areas operation. This can include anything from the surgeon who performs coronary artery bypass to the neonatal intensive care nurse who directly cares for struggling newborns to the physical plant worker who makes sure all the lights in the building stay on and the operating room is maintained at a certain temperature. As an overall institution the thought of identifying risk and liability within the organization can be quite overwhelming and daunting. The best way to approach this is to break things down into specific issues or areas and focus on one thing at a time, with the overall goal to be improving quality and reducing risk to the institution. This paper will focus on three specific scenarios that represent an area of risk and liability for the institution. These three scenarios will include the safety of blood transfusions within a hospital, dealing with power failure risks within a hospital and using operational checklists to improve employee efficiency, consistency and reducing the human factor of making mistakes. Scenario one is to be specific to a scenario involving patient care and safety. The specific discussion here will be the use of blood transfusions within the hospital setting. This is a procedure that has been done for dozens of years even as medical science has made tremendous progress. The reality is that science just hasn’t found a synthetic way to carry oxygen through the blood stream and blood infusions still remain today the best way to do just that. While the best procedure out there for this, blood infusions don’t come without their risks and financial impacts. A study by the University Healthcare Consortium analyzed over 29,000 blood transfusions over a 7 year period from 2003-2009 (Williams, 2011). These transfusions ranged anywhere from scheduled routine outpatient surgeries to unexpected traumatic injuries brought in through the emergency room. The analysis identified many risks associated with blood transfusions, including poorly trained staff, lack of trained staff, minor hemolytic allergic reactions all the way up to severe anaphylactic reactions. The study was even able to identify that the time of day that the transfusion was performed could positively or negatively affect the patient outcome. So, while this can be a lifesaving procedure a blood transfusion should not be taken lightly. Another study published in the April, 2012 issue of the professional journal Anesthesiology was conducted by Johns Hopkins Hospital analyzing the outcomes of over 3000 patients who received blood transfusions. This study reveals that measuring the hemoglobin level in patients can be an accurate indicator of when a patient should receive a blood transfusion. This is a significant study because it could have a positive effect on both patient outcome and the financial costs involved in giving a blood transfusion. The Johns Hopkins study revealed that patients were being given blood transfusions when they really didn’t need it. A normal hemoglobin level ranges between 12-14 and the study reveals that a level as low as 7 or 8 is safe. Prior to this study the leading specialty societies, including the FOCUS research group (Functional Outcomes in Cardivascular patients Undergoing Surgury), set parameters that transfusions were definitely needed if the hemoglobin was below 7 and probably did not need a transfusion if the hemoglobin was 10 or above. But nobody ever set any parameters on what to do if the hemoglobin ranged between 7 and 10. This left the physician to decide when to start a blood transfusion. The Johns Hopkins study revealed that because of this most physicians always erred in the side of â€Å"safety† by ordering a transfusion any time the hemoglobin was at or below 10. But the Hopkins study has now determined that a hemoglobin of 7 or 8 can be considered safe. Giving blood to patients who have a hemoglobin above 7 shows no real benefit and truth is that the risk due to side effects may actually be increased. Remember that this procedure has been in place for decades and this new research was a game changer. The benefits to a hospital from the Hopkins study are tremendous. The study revealed that the costs to the hospital for one unit (300ml) of blood can cost as much as $1,100. While the donor gives the blood for free it must be analyzed for toxic diseases including Hepatitis and HIV (IMVS 2012). The blood is then separated into red blood cells, platelets and plasma, to be distributed specifically where needed. Include the storage and transportation costs and you can start to understand why blood is so expensive. Only giving blood when truly indicated has been shown to reduce the use of blood by up to 66% per institution with no change in outcomes to the patient including â€Å"length of stay, heart attack, stroke, death, and even the ability to walk† (Clark, 2012). This is where the HIM professional can come into play. The HIM can take research articles like this or even acquire their own research and then do training to educate the hospital staff. The Hopkins tudy indicated that when the HIM spoke directly with the surgeons and showed them the research there was a tendency to accept the new mindset and start delaying blood transfusions until the patient hemoglobin dropped below 7. This resulted in extra blood supply for those patients who truly need it and a significant reduced operating cost to the institution. It’s the HIM’s job to establish or change procedures that reflect changing and beneficial modalities and to get the information out to the hospital physicians and staff so they can help reduce institutional costs and ultimately benefit their own livelihood. This process can be done for hundreds or even thousands of processes currently in place within a hospital institution. Now we move on from our blood transfusion scenario to one involving the infrastructure of the hospital. The specific example to be used here is the hospital power supply. Hurricane Sandy revealed for many healthcare institutions in the north eastern United States that they were not as prepared as they thought. Some of these hospitals did have emergency generators in place, but they did no good for providing electricity when they were located in the basement of the building submerged in water from the flood. Sometimes it takes a real catastrophe for us to truly see how prepared we really are and how to improve. It’s crucial that a hospital be able to maintain electricity at all times. Almost every part of the building depends on electricity for staff to function and take care of their patients. Without electricity the lights won’t work, the furnace and air conditioners won’t work, the pumps won’t pump water through the pipes, ventilators will stop working and elevators won’t even be able to transfer sick patients between floors. These are some examples, but surely you can find many more failures if you look hard enough. For this reason, it is important that a hospital evaluate and have a plan in place to reduce the possibility of losing electricity and also have a plan in place if all safe guards fail. The concerns can truly be specific to a geographic area. For example, a hospital in the Midwest may not have to worry much about being hit by a hurricane, but it could be hit by a tornado. I spoke with hospital administrator Robyn Mazzolini at Advocate Lutheran General Hospital in Park Ridge, Illinois regarding how their hospital has addressed the issue of maintaining electricity to the facility. Robyn indicated that there were three specific issues of highest concern. They included the loss of power from the local utility company, flooding of the existing generators in the basement of the main hospital and damage from tornados. Robyn indicated that about ten years ago the hospital installed a generator in the basement of the main building that is capable of providing power to the entire ten story hospital, including all lights and HVAC. The problem was that the unit was very large and heavy and, as a result, had to be installed in the basement. A second generator was installed right next to the first to act as a backup generator should they lose power from the power company. These generators are also designed to operate on both natural gas or stored gasoline in the event the gas lines become compromised. This seemed to solve 90% of the scenarios that could come up. Then the neighboring community experienced a flood that shut down utilities for days and a few weeks in some areas. The hospital was concerned that they would be shut down in the event of a flood in their generator rooms in the basement. So, they added a third generator inside a remote area on the south perimeter of the hospital. This generator is located inside a concrete structure three floors above ground level. This is a smaller generator that will run specific areas in order for the hospital to stay functional. For example, this generator only supplies certain lights so that there is always enough visibility to operate. It also is designed to operate all electronics in the operating rooms, emergency room and critical care areas so there is no compromise to patient care. On the remaining medical floors there is one or two red colored electric outlets that operate off this third generator. In addition, this generator is not connected in any way to the basement level so that it can power the upper floors of the hospital even if the ground level is buried under 20 feet of water. The third generator is also enclosed in walls of reinforced concrete with the thought that it could withstand a tornado. It was the HIM and the HIT who were involved in the process to help determine the essential power needs of the hospital. The HIM evaluated the staff and the various facilities to determine that they could function treating most patients as long as there was one dedicated outlet within each patient room. The HIT also identified that the operating rooms, critical care areas and emergency room pretty much needed full power capabilities no matter what. The HIM and facilities managers both have a critical role in communicating the needs of the patients, staff and hospital to the engineers, administrators and building designers. And while Lutheran General Hospital was able to spend millions to prepare for a power disruption and pretty much covered every scenario, they still need to be ready to operate should all power to the facility cease. It is the HIM who can evaluate the various areas of the hospital based on resources, staffing and needs to determine the most critical issues to address first in the event of a total power failure. This takes a lot of time and energy and is probably an evolving process as conditions and resources change. But, in the end, it is the HIM professional who is able to connect all the dots to help bring the best solution for the organization. The third and final scenario presented here is related to staffing and how the HIM can help reduce risk and liability when it comes to the behavior of hospital staff. Hospital employees by their human nature are susceptible to making mistakes or forgetting to do certain things in the course of their patient care. Every health care institution, and any other business for that matter, strives to find ways to provide the best service possible to its customers. The challenge in improving service and reducing liability is finding a process that actually works. Ohio State University Medical Center found a way to improve the quality and consistency of patient care from its medical professionals. Ohio State did created a system of check lists to be used by hospital staff in their particular department and area of responsibility. So, there were many different checklists, but each employee only had one checklist that they were specifically responsible for. The University compared the outcome of over 3000 patients both before the check lists were used and then an additional 3000 plus patients after the checklists were used. The results were staggering. The study showed, for example, that the surgical death rate of patients decreased 47% when check lists were used compared to none used. Surgical infection rates were found to have decreased by 45%. These are dramatic results that clearly show a huge improvement in patient care and reduction of risk and liability. A possible cause for such improvement is that using checklists prevented medical staff from skipping a stop that could have caused a serious complication or infection. The aviation industry has proven that checklists work. When a pilot has a sudden engine failure there is plenty of room for panic and impulsive missed steps. This is why pilots are trained to immediately pull out their check lists, even if they have them memorized, and go systematically down the list to make sure that all steps are completed as indicated. This following of check lists has proven to significantly improve survivability of the aircraft during an emergency. The use of check lists seems to hold true for the health care industry, too, as the data clearly shows. The HIM plays a critical role in the development and rollout of these check lists to the various departments. The HIM must involve the hospital staff, managers and physicians in the process of developing a check list that easily and safely covers the needs of both the patient and the hospital. Once developed the HIM must develop and take the time to train the staff so that they understand the intent of the check lists as well as the individual steps and why they are to be completed. This training will ensure that the employees buy into the use of the check lists and choose to use them consistently with every patient. Theoretically, this should provide the same process and the same desired outcomes each time. Besides implementing the checklist program through training the HIM also plays an important role in developing a policy that says how the plan is to be implemented. Employees appreciate it when everything is clearly spelled out of what is expected of them. The HIM is in a position to deal with both the hospital administrator and the front line nurse who is physically caring for patients. By analysing the levels of motivation with the help of Maslows Hierarchy of Needs, it was found that the top three levels of needs go dicey in case of Apple employees. However, there is no denying the fact that Apple has mastered in the concept of change. Whether it is about training the employees for change or it is about managing the changes in distribution strategy, Apple has successfully managed changes both within a s well as outside the organisation. My recommendations to the company, particularly to Steve Jobs, are: No wonder apple has no match in its innovation. But things can further improve provided that employees are given more freedom to express their thoughts. Moreover, if instead of Stick, Jobs can manage with some positive motivation, it can do wonders for the company as far as employee loyalty is concerned. In our lab, we had three main objectives. First, we evaluated our data on monohybrid and dihybrid corn cross seed counts against Mendel’s theoretical expectations of independent assortment and the segregation of alleles. Next, we used the Hardy-Weinberg Theorem to provide a theoretically expected value for allele frequencies for single human gene traits. Lastly, we dealt with Drosophila melanogaster and we examined red and white eye alleles to determine if this gene is sex-linked or autosomal.   Ã‚  Ã‚  Ã‚  Ã‚  During the mid 1800’s Mendel bred garden peas to study inheritance. He choose these plants because of their well defined characteristics and the ability to be grown and crossed (Campbell, 1996). Mendel followed traits for the P generation, F1 generation, and F2 generation. The P generation is the original true-breeding parents. Their hybrid offspring is the F1 generation, the first filial. The F2 generation is the second filial and is the self- pollination of the F1 hybrids.
